Cardiocom, Medtronic's patient monitoring services business, announces partnerships with home health care companies
DUBLIN and MINNEAPOLIS - Feb. 2, 2015 - Medtronic plc (NYSE: MDT) announced today a partnership agreement between its Cardiocom business and LHC Group (NASDAQ: LHCG), a U.S. provider of home health, hospice and comprehensive post-acute healthcare services, to provide integrated clinical telehealth services and medical technology to their patients. This agreement adds to the growing list of home health care organizations that are now customers of Cardiocom, Medtronic's Patient Monitoring Services business. Four of the five largest home health care companies in the United States are now Medtronic customers.

The agreement enables Medtronic to provide telehealth devices for LHC Group's existing care transitions program, T3 -Transitions, Training and Triage - to support their disease management approach that has been clinically proven to reduce emergency room visits and prevent avoidable readmissions. Specifically, LHC Group will be utilizing the COMMANDER FLEX remote patient monitoring devices, which simplifies home telehealth management and education for patients with complex conditions such as heart failure, hypertension, COPD, and diabetes. The COMMANDER FLEX system allows LHC to conduct the daily monitoring of symptoms and vital signs to anticipate events, assess risk and communicate with patients before an acute event such as a hospitalization, providing the most appropriate and cost effective care to their patients.
Medtronic's innovative home telemonitoring equipment and telehealth services enable health professionals to identify symptomatic patients and intervene early to prevent unnecessary hospitalizations.
