Controversies in ICD Therapy

At ACC.09, the controversies in implantable cardioverter defibrillator (ICD) therapy included:
  • Misuse of SCD-HeFT data
  • Prophylactic ICDs in the elderly and patients with comorbidities, and
  • ICD therapy in the era of genetic testing

At ACC.10, the controversial topics in ICD therapy include:
  • Update in risk stratification for sudden cardiac death
  • Appropriate patient selection for cardiac resynchronization therapy
  • Management of patients with frequent appropriate and inappropriate ICD shocks
  • Update in lead extraction: indications, techniques and management

In many ways, the field of electrophysiology is in its infancy, or at least approaching adolescence. The plethora of new drugs and technology and the ever-improving understanding of arrhythmias keeps EPs on the learning curve. Just last year, the Heart Rhythm Society released updated guidelines for lead extraction. It used to be that EPs would leave defective leads inside the patient, while implanting new leads. But that protocol is becoming increasingly unpopular as our understanding of infection improves.
