Missouri heart failure clinic recognized for best practices
Mercy Clinic’s Ambulatory Heart Failure Care Management Team has been recognized as a Stronger Hearts Best Practice facility by the National Forum for Heart Disease and Stroke Prevention, reports the emissourian.com.
The Washington, Missouri-based clinic treats high-risk heart failure patients and assists in their transition from a hospital to an ambulatory setting. They address the mental, social and physiological factors of a patient by organizing home visits by nurses and social workers.
The committee that decides the recipient of the best practice award is led by Clyde W. Yancy, MD, chief of cardiology at Northwestern Medicine in Chicago.
“It’s exciting to have our program recognized, but even more exciting that we’re able to keep our patients healthier using team work and better coordinated care,” said Carolyn Koenig, MD, medical chair of quality for Mercy Clinic in the eastern Missouri region, which includes Mercy Hospital Washington.
