Medical director of the Allina Health Minneapolis Heart Institute (AHMHI) Cardiology Clinic and the Comprehensive Cardiovascular Care Coordination Center (C5) at Abbott Northwestern Hospital 

Jonathan is reworking the geography of cardiovascular expertise by proving that elite specialty care need not be confined to a single building—or even a single state. 

Launched in 2024 under Dr. Urbach’s leadership, C5 is a first-of-its-kind, 24/7 cardiovascular command center that connects clinicians across Minnesota and western Wisconsin to real-time cardiology consultation and standardized care guidance through the AHMHI mobile app or a phone call. The innovation earned recognition at the 2024 Allina Health Performance Improvement Symposium and the 2025 Health Catalyst Analytics Summit—notable achievements, especially for a cardiologist whose career is still in its early stages.  

At 38, Jonathan brings formidable academic credentials to his operational work. Holding board certifications in cardiovascular disease, echocardiography and cardiovascular CT—including Level 3 CT verification—he serves as site principal investigator for multiple Phase III cardiovascular outcomes trials. He also has authored peer-reviewed research on artificial intelligence in coronary CT angiography, heart failure therapeutics and imaging-derived risk stratification. 

Beyond the lab and clinic, Dr. Urbach shapes the next generation of cardiovascular physicians through the Minneapolis Heart Institute Foundation’s Education Board and as an invited speaker for cardiology fellows and Grand Rounds. Patients consistently rate him among the region’s best, and he was named a "Top Doctor" by a Twin Cities publication in 2026.
