Payor/provider medical home piloted in Pittsburgh
Pittsburgh-based payor HealthAmerica and Preferred Primary Care Physicians have launched a pilot program to provide coordinated and patient-centered primary care through improved communications with patients, physicians and care teams.
Preferred’s 32 board-certified physicians and five physician extenders specialize in internal medicine and family practice. Preferred, which has 14 practices in the South Hills area south of Pittsburgh, and three locations in Uniontown in Fayette County, includes cardiac testing, sleep disorders and physical therapy outpatient settings.
The pilot program, launched Sept. 1, leverages the advanced medical home model established by the American College of Physicians. The medical home team approach is led by a physician who provides continuous and coordinated care throughout a patient's lifetime to maximize health outcomes. Care delivery is facilitated by the patient's physician and includes subspecialty care, hospitals, home health agencies and nursing homes.
